When to use this form
This form can be used by either a paying parent or a custodian who is a party to a formula assessment of child support.
Only complete this form after you've checked that the assessment could not be changed by any other option - you can read about other options in our guide Helping you to understand child support reviews (IR175).
What you will need
- your IRD number
- information to support your application. We recommend you complete a Child support administrative review - statement of financial position (IR178) form and send this with your application.
