When to use this form
Print your first six children's details at Question 9 of the registration form (FS1) and use this form if you have more than six children you are entitled to receive working for families tax credits for.
What you will need
You will need all of the children's details including date of birth, date the child started living with you (if different from the date of birth) and the children's IRD numbers.
