When to use this form
- If you are receiving a Work and Income benefit that includes a sole parent rate, you are unable to cancel a formula assessment for child support
- this form can only be completed by a custodian
What you will need
You need to decide if you want us to collect any outstanding monies, that you may be entitled to, and or penalties that the paying parent may have.
