About this form
The non-deduction notice allows employees to ask their employers to cease making deductions from their pay for KiwiSaver contributions. The notice is required under the KiwiSaver Act 2006 (section 112B(1)).

When to use this form
KiwiSaver members who have reached the age of 65 and have been a KiwiSaver member for 5 years or more, become eligible to withdraw their KiwiSaver savings. At any time after reaching that date an employee can stop their KiwiSaver deduction from their salary. This decision is optional.
The first members reaching the age of 65 who have been in the scheme 5 years happens from 1 July 2012 onwards.
What you will need
- Employee's name and IRD number
- Employer's name and address - this is not essential as long as the notice is given to the employer with the employee's name, IRD number and signature.

The employer will retain the notice as part of their records.
