About this form
If you are an authorised bank you must commute the duty on cheques issued to your customers quarterly.

When to use this form
This form must be completed quarterly by all banks authorised to commute the duty on cheques issued to their customers.
What you will need
The signature of the bank's general manager, an accountant or another senior administrative officer in the bank's New Zealand head office.

Please print, sign and post the completed form and payment within 21 days to your nearest Inland Revenue office.
The bottom copy will be returned when receipted.
