When to use this form

Use this form to apply for an IRD number either for yourself or a child in your care.

If you need to apply for a business IRD number, use the IRD number application - non-individual (IR596).

What you will need

You will need to provide originals of two identification documents as well as a legible photocopy of those documents

You will find the details of these required documents on the front page of the IR595 form.

After you finish

Please print, complete and sign your IR595. Take the form with your identification documents for an 'in person verification' to an authorised Inland Revenue agent. These include:

• Automobile Association (AA) Driver Licensing Agents
• PostShops and selected New Zealand Postshops

We will send confirmation of your IRD number within 8-10 working days of receiving the application form.

You may wish to keep a copy of this form for your personal records.
