About this form

For periods from April 2009 onwards. This is a form and payment slip to record and pay the deductions made from your employees' pay (includes ESCT).

This form can be completed on-screen by typing content directly into the PDF document. Once you have completed the form it is important that you print it out as you will not be able to save the information you enter into the form.

When to use this form

Use this form to record and pay deductions from your employee's pay to us.

What you will need
  • your IRD number
  • the period ended (period you are paying PAYE for)
  • the amount of PAYE deductions
  • the amount of child support deductions
  • the amount of student loan deductions
  • the amount of KiwiSaver deductions
  • the amount of KiwiSaver employer contributions
  • the amount of ESCT
  • total deductions being sent in

You'll also need to attach payment or advise if payments have been made electronically or at a branch of Westpac.

Please note that this form no longer has a section for you to retain. If you need to keep a copy for your records, please make a copy and then send the original form to us.
