When to use this form

Use this form to work out your, and/or your spouse or partner's income and business adjustments for the tax year. The adjustments will help us work out your Working for Families Tax Credits entitlement correctly. They don’t change the amount of income you need to show in your Individual income tax return (IR3).

What you will need
  • your IRD number
  • details of the following business income adjustments:
    • depreciation recovered on sale of any buildings used in business
    • income equalisation reserve scheme refunds (if deposits were made in the 2002-2003 or earlier years)
    • any income spread to the income year
    • depreciation recovered on sale of rental buildings
    • allocated income from a portfolio investment entity (PIE)
  • close company shareholder details.
