About this form
This form is for applying for paid parental leave payments, if you're an employee taking parental leave from your job. It also provides information on paid parental leave and who is eligible.

When to use this form
Use this form to apply for paid parental leave once you've confirmed with your employer that you qualify for parental leave from your job. For information about parental leave call the Department of Labour on 0800 20 90 20.
You can apply for paid parental leave before your parental leave commences, or at any time while you're on leave. You must however apply for the payments before you return to work or resign.
If you're self-employed and are applying for paid parental leave, you need to use a Paid parental leave application for a self-employed person (IR888) form. You can get this form from "Forms and guides".
What you will need
- Your IRD number - if you do not have this call us on 0800 227 774.
- Birth date or expected due date of your child (or adopted child)
- Your bank account number.
This form has two sections that must be filled in. You (the applicant) need to fill in the applicant's section, and then give the form to your employer to fill in the employer's section.
