When two or more companies join together this form should be completed to tell us about the amalgamation. It must be completed by a representative of the company resulting from the amalgamation.

When to use this form
Complete this form to tell us that there has been an amalgamation within 63 working days of delivering the almagamation documents to the Registrar of Companies.
If the resulting company is a newly incorporated company you will need to fill the IRD number application form for non-individuals (IR596).
What you will need
To successfully complete and submit this form you will need:
- Your company's current IRD number.
- Previous names and IRD numbers of the amalgamating companies.
- The date of amalgamation.
- To attach the amalgamation certificate.

Please print, sign and attach your completed amalgamation certificate and post to your nearest Inland Revenue office.
Please make sure you submit this form within 63 working days of delivering the amalgamation documents to the Registrar of Companies.
