Complete this form at any time up to the due date for payment of your last instalment of provisional tax.
Send the completed form to Inland Revenue by that date.
Note: If you choose to estimate your provisional tax, you cannot change to the standard option for that year.
Estimating your provisional tax may influence any interest you might have to pay.
What you will need
You will need your IRD number and the figure you estimate will be your provisional tax amount to pay for the year.
