When to use this form
This form is for an employee to provide their details to an employer if they are:
- starting new employment
- an existing employee who wants to opt into KiwiSaver
- a KiwiSaver member who wants to change their contribution rate
What you will need
All employees who are automatically enrolled or opt into KiwiSaver should give their name, IRD number and address to their employer.
