This form is for employers to enrol eligible employees in KiwiSaver.

When to use this form

This form is for employers to notify Inland Revenue about:

  • a new employee being enrolled automatically in KiwiSaver
  • an existing employee who opts in.
What you will need

All employees who are automatically enrolled or opt into KiwiSaver should give their name, IRD number and address to their employer.

After you finish

Please print, sign and post the completed form to your nearest Inland Revenue office.

You may wish to keep a copy of this form for your personal records.
