KiwiSaver employee details KS1

This form is for employers to notify Inland Revenue about:

  • a new employee being enrolled automatically in KiwiSaver
  • an existing employee who opts in.

All employees who are automatically enrolled or opt into KiwiSaver should give their name, IRD number and address to their employer.

After you finish, please print, sign and post the completed form to your nearest Inland Revenue office.
