- IRD number and position title (as the ‘valid owner’)
- organisation’s name.
You’ll also need the executive office holder’s:
- full name
- postal address
- phone number
- IRD number.

You can fill in the form on your computer then print it out.
Sign your IR401 and attach your supporting documentation
The documentation should support the position of the person you’re appointing into the executive office holder role.

What happens next
We’re able to process:
